**Please see the information below regarding the 2023-2024 school year**

New York State Student Health Requirements:
1. Physicals: Physicals completed by a doctor are required for new students and all students entering 9th and 11th grades. New York State requires that a physical exam be on file in the Health Office within 30 days of the new school year. Physicals dated after 9/7/2022 may be accepted for the 2023-2024 school year. Physical exams must be completed on this form: Health Exam form 2023-2024


2. Immunizations: 
All 12th graders are required to have a minimum of 1 Meningitis vaccine on or after the age of 16 years. All immunizations must be up to date within 14 days of starting school. It's best to check with your doctor to see whether or not your child needs the vaccine. Students who are not up-to-date will not be allowed to attend school until they are vaccinated. 3. Medications:
According to NYS law, any medications administered at school, even over-the-counter medications (cough drops, Tylenol, Advil), require written permission from the physician and the parent or guardian. They must be unexpired, in their original container, and must be brought to school by an adult to ensure safety. Inhalers need a pharmacy label on the inhaler itself, not just the box. Medication orders including epi-pens, inhalers, other self-carry orders, and daily medications must be renewed each fall.  **Over the counter medications such as cough drops, Tylenol, or Advil are not stocked in the health office. We do not accept forms with standing orders for these medications**

4. Allergies/Asthma Requiring use of emergency medication:
Students who have food/insect/latex/environmental allergies or asthma requiring the use of an emergency medication such as an epi-pen or rescue inhaler must have an emergency care plan on file. The emergency care plan must be renewed each fall. The emergency care plan forms can be found here.

**Sports Clearances**

For the most updated information on sports registration, please visit the Athletics webpage. All registrations are completed online through FamilyID. All physicals, medication orders and emergency care plans must be uploaded to FamilyID. Even though a copy of the physical may be on file in the health office, no hard copies will be accepted for your clearance. Although there may be a physical attached to a prior registration, please upload a current physical to make the processing more efficient. **Additional Important Information**

Please do not send your child to school if they are experiencing any of the following:
- Vomiting or diarrhea in the last 24 hours. Students must be symptom free for 24 hours before returning to school.
- Fever of 100.4 or above. Student must be fever free for 24 hours without medication before returning to school.
- Diagnosis of strep throat. Student must be on medication 24-48 hours before returning to school.

*Latex Allergy Alert*
There are students with severe latex allergies. Please DO NOT bring any latex products into this school. Some items that can contain latex are: rubber gloves, medical products, latex balloons. Thank you.
